Hormone replacement therapy (HRT) replaces the hormones that naturally decline during menopause — primarily estrogen and progesterone, plus testosterone when indicated. Hormone levels change with age
Illustrative curve. During perimenopause, estrogen can swing between peaks above normal cycle values and sharp crashes — symptoms are often worst here. Progesterone falls fastest as ovulations become less frequent. Testosterone declines gradually from the 20s, independent of menopause.

What does the research say?
Findings from peer-reviewed publications.
NAMS 2022 position statement: HRT is the most effective treatment for vasomotor symptoms and protects bone density in women under 60 or within 10 years of menopause.
Transdermal estradiol carries a lower venous thromboembolism risk than oral estrogen — meta-analysis of observational and randomised studies.
Micronised progesterone has a more favourable risk-benefit profile for breast and cardiovascular health than synthetic progestins.
