Bioactive (free) testosterone is the fraction not bound to SHBG or albumin — it is what reaches tissues and drives symptoms. Because SHBG rises with age, free testosterone declines faster than total. Two men with identical total testosterone can therefore feel completely different.

    Premium health check with 70+ biomarkers, DEXA scan and physician consultation. We measure total and free testosterone, SHBG, LH, FSH, estradiol, prolactin, PSA, haematocrit and blood lipids.
  2. 02Lifestyle optimisationIf needed0–3 months
    For borderline-low values we optimise sleep, body fat, strength training, zinc and vitamin D status, and stress management before considering medical treatment.
  3. 03TRT initiationMonth 1
    Short-acting injections (intramuscular or subcutaneous) provide stable levels and allow precise dose titration. We choose dosing and route based on your goals and lab values.
  4. 04Close follow-upEvery 3 months
    Initially we monitor haematocrit, estradiol, PSA, blood lipids and clinical symptoms every 3 months to fine-tune the dose and identify any side effects early.
  5. 05Long-term maintenance1–2 / year
    Once treatment is stable we follow up 1–2 times per year with full blood count, hormone panel and clinical evaluation. This is a lifelong part of the treatment.
