GLP-1 treatment and body composition — what does the research say?

GLP-1 drugs like semaglutide and tirzepatide produce significant weight loss. But how is muscle mass affected? Read about evidence, risks and optimizing results.

GLP-1 receptor agonists like semaglutide (Ozempic/Wegovy) and tirzepatide (Mounjaro) have revolutionized the treatment of overweight and obesity. Clinical studies show weight losses of 15–25% of body weight. But weight loss doesn't equal health gain if it comes at the expense of muscle mass.

How do GLP-1 drugs work?

GLP-1 is a gut hormone that regulates appetite, satiety and blood sugar levels. GLP-1 receptor agonists mimic this hormone, providing reduced hunger, slower gastric emptying, and improved insulin sensitivity.

The challenge: muscle mass loss

If you don't actively focus on resistance training and adequate protein intake (1.6–2.2 g per kg body weight) during GLP-1 treatment, you risk losing a significant proportion of muscle mass instead of fat. The consequences are serious: impaired metabolic health, reduced bone density, decreased functional capacity and fitness, increased frailty and injury risk, and lower basal metabolism — making it significantly harder to maintain the new weight long-term. That's why a structured, data-driven approach is essential to ensure GLP-1 treatment delivers real health gains — not just a lower number on the scale.

How we optimize your GLP-1 treatment

  • Regular DEXA scans to monitor body composition (fat vs. muscle mass)
  • Structured strength training program with continuous access to your own personal trainer
  • Optimized protein intake (1.6–2.2g per kg body weight)
  • Hormone optimization as needed (TRT, HRT)
  • Blood test follow-up of metabolic markers
  • Continuous access to physician within 24 hours
  • Ongoing monitoring with dosage adjustments and combinatory treatment recommendations

DEXA scanning before and during GLP-1 treatment is crucial to ensure you're losing fat — not muscle mass.

The Revi model: data-driven, not guesswork

We measure before, during and after. A health check with DEXA scan and blood tests gives you a complete picture of your metabolic health, body composition and hormone profile. Your doctor uses this data to optimize your GLP-1 treatment and minimize muscle loss. We always combine GLP-1 treatment with personalized support for diet and exercise through a personal trainer — to maximize fat loss and preserve muscle mass.
